Registration Fees – €40 per child / €100 for 3 or more children (Siblings)

* I understand that Nazareth Mar Thoma Church would use the photographs/videos of the child taken during VBS to be used for promotional purposes, printed publications, internet posts including social media and other media sources. * I authorize Sunday School office bearer's or designated medical professionals to administer emergency medical assistance in the event that I cannot be reached. * I release and hold harmless ‘Nazareth Mar Thoma Church’ and it’s volunteers from any liability, past or future, fully and completely in connection with the use of images of my child and for any injury which may be suffered by my child arising out of or in any way connected with participation in this VBS program.
